Notice to Parents and Guardians of 6th grade Students Promoting to 7th grade

In order to simplify the 7th grade registration process, if possible, please submit your child’s immunizations during this school year by one of the following methods prior to June 7, 2022:


1. Email immunizations to the School Nurse: via email to [email protected]  
2. Drop off a copy of immunizations at Yorba Linda Middle School.
3. Fax immunizations to: 714) 996-2752 

If you are not able to submit completed records by June 7, 2022, you will have an opportunity to submit them to your child’s middle school at registration.

PYLUSD understands that making routine well-child visit appointments may be difficult at this time. However, we want to give you ample time to have immunizations completed.
